Total cholesterol minus HDL: captures atherogenic lipoproteins including LDL.
What it is
Non-HDL cholesterol in mmol/L. Useful when triglycerides are raised or LDL is hard to calculate.
Non-HDL cholesterol is total cholesterol minus HDL cholesterol. It captures cholesterol in LDL, VLDL, and remnant particles, the apoB-containing, atherogenic lipoproteins, in one simple subtraction. Reported in mmol/L.
Why it matters
Used in UK lipid pathways alongside CVD risk assessment. It includes remnant particles that still matter for risk.

UK lipid pathways often use non-HDL alongside CVD risk tools, especially when triglycerides are raised or calculated LDL is unreliable. Targets depend on whether you are in primary or secondary prevention and on clinician judgement.
Lifestyle (diet pattern, alcohol, activity, weight) and prescribed medicines are the evidence-based levers, not unregulated “cholesterol detox” products.

How it affects the body
By subtracting HDL from total cholesterol, non-HDL approximates the cholesterol burden in apoB-containing particles.
ApoB-containing particles can enter artery walls, become retained, and fuel chronic plaque inflammation. Non-HDL approximates that particle cholesterol burden without needing a separate apoB assay on every panel.
For athletes
Practical for athletes who struggle to fast perfectly before panels, still interpret with a clinician for treatment thresholds.
Masters and recreational athletes can have reassuring HDL yet raised non-HDL from genetics or diet. Fasting helps triglyceride-related calculations; for non-HDL itself, consistency of timing across season checks matters most.
